Connected Care

As part of this interactive experience, attendees answered questions around their biggest challenges. Here are the results*:

2. What are the greatest challenges to care management your health system has faced outside the hospital?
 

  • 11% High patient utilization of emergency services post-discharge
  • 16% Reducing costly readmissions
  • 18% Staying connected to patients after discharge in order to monitor their progress

3. What issues prevent care teams at your health system from confidently making treatment decisions?

  • 16% Difficulty consolidating data

4. What has been your most challenging issue as your health system works towards connected value-based care for patients at home?
 

  • 23% Building an aligned care pathway to manage patients at home with targeted conditions, like COPD

  • 26% Loss of visibility and connectivity once patients leave the hospital
